Diabetes Self-Management Education (DSME) Program: A series of four classes held monthly Tuesday evenings and daytime Thursdays. The program includes a one-hour assessment to be performed prior to start of classes. This program is recognized by the American Diabetes Association and based on national standards for DSME programs. Participants must obtain an order from his/her physician, who is then informed of your progress. Fees to participate in this program are typically covered by Medicare and other health insurance plans.
Individual DSME consultations by appointment: Intended for people unable to attend group classes; people with learning, language, hearing or vision problems; people with Type 1 diabetes; and for those needing instruction regarding the use of insulin or intensive insulin regimens. Participants must obtain an order from his/her physician, who is then informed of your progress. Fees to participate in this program are typically covered by Medicare and other health insurance plans.
Beyond the Basics: This two-hour class is a one-year follow-up program for those who have completed the initial DSME program. Others may consider scheduling a one-on-one appointment with a nurse and dietitian for an update. Participants must obtain an order from his/her physician, who is then informed of your progress. Fees to participate in this program are typically covered by Medicare and other health insurance plans.
Medical Nutrition Therapy: Sessions scheduled by appointment with a Registered Dietician who is also a certified diabetes educator (CDE) on topics related to nutrition. Topics include Carbohydrate counting, reducing fat in your diet, weight-management, exercise, meal plan development, and nutrition for kidney disease. Participants must obtain an order from his/her physician. Diabetes and kidney disease are topics covered by Medicare and other insurance plans. Some diagnoses may not be covered.
Gestational Diabetes Program: This specialized diabetes education program teaches self-management techniques to mothers-to-be in a personalized, one-on-one session. The nurse and dietitian work with the expectant mom to improve her blood glucose control and teach skills to ensure a healthy pregnancy. Individual appointments are typically made for two-four sessions. Participants must obtain an order from his/her physician, who is then informed of your progress. Fees to participate in this program are typically covered by Medicare and other health insurance plans.
Your Game Plan for Preventing Type 2 Diabetes: This two-hour diabetes prevention class provides an overview of pre-diabetes, self-risk assessment, lifestyle modifications and risk reduction. A nominal fee is required. Program instruction will be given by both a nurse and a dietitian.
Diabetes Support Group: Held the 2nd Wednesday every month at 7 p.m. in the St. Mary Mercy Hospital auditorium. Guest speakers address topics of importance to those with diabetes. Pre-registration is not required and there is no fee to attend.
