Social Work and Case Management

Social workers are available to meet with patients individually or as a family regarding any concerns experienced due to illness. They assist in considering difficult care decisions or provide information about community services. Our social workers can help find appropriate resources involving care for the elderly, counseling, domestic abuse and similar situations.

Case managers meet with patients and other members of the health care team to evaluate the health status of patients and to anticipate what types of post-hospital services may be needed. Transportation Assistance

Assistance with transportation is available on a first-come, first-served basis to individuals coming to St. Mary Mercy Hospital for certain health care services.
