Patients can schedule diagnostic testing by calling the Central Scheduling Department at 734.655.2961, or 800.494.5805 (out-of-area.) When calling, you should have your physician’s order available. The scheduler will need to know the name of the test and the diagnosis the physician provided on the order. The scheduler will pre-register you for your visit. You will be asked to give your personal, demographic, and insurance information. It is a good idea to have your insurance cards available when calling.

When you arrive at the Outpatient Registration desk, a registration clerk will greet you. If you are not pre-registered, the registration clerk will obtain basic information from you – e.g. your name, telephone number, date of birth – and will request information about you, your family, physician, employment and health insurance. You will also be asked to sign a consent form giving permission to receive medical care. A parent or legal guardian must sign a consent form for patients being registered under 18 years of age.

In providing you and your family with comprehensive, coordinated and compassionate care at St. Mary Mercy Hospital, each patient now receives a Patient Courtesy Card at his/her first registration. This card contains your name, date of birth, your medical record number and your Patient Identifier Number, used by St. Mary Mercy Hospital and other Michigan Trinity Health System facilities. Presenting this card every time you visit St. Mary Mercy for health services streamlines your registration experience.
